MEDICAL DIRECTOR – Full-time

SUMMARY:  A Family Practitioner to provide leadership in the delivery of quality medical care to all patients in accordance with the goals of Lotus Health and Wellness, share in the administrative functions that directly impact medical services and collaborate with management in overall planning and budget activities.  Function as a provider to a panel of patients.

QUALIFICATIONS: Graduate from an Accredited Medical School; Valid NYS Medical License required; DEA Registration; Board Certified in Family Practice or Pediatrician; BCLS or ACLS/PALS Certifications; ability to attain privileges in community hospitals; must countersign collaborative practice agreement for nurse practitioner.   Five (5) years experience in a leadership role in a primary care setting, working with indigent populations & community based programs. Criminal background check & drug screening required.

ANNUAL SALARY:  Negotiable, Paid Benefits (Medical, Dental, Life Insurance, Disability, Paid Time Off).

PHYSICIAN – Per-diem

 SUMMARY:  A Family Practitioner to provide primary medical care to patients including diagnosis, treatment, consultation and follow-up in a strictly outpatient setting.  Provider will be expected to provide high quality of care for all patients on their panel, and have a desire to be an integral part of a developing team of clinicians dedicated to changing health care delivery in this area. Provider will also be expected to participate in QA/QI initiatives.

QUALIFICATIONS:  Graduate from an Accredited Medical School; Valid NYS Medical License required; DEA Registration; Board Certified in Family Practice or Pediatrician; BCLS or ACLS/PALS Certifications; ability to attain privileges in community hospitals; proficient in electronic health record (EHR); 2-3 years clinical experience; excellent time management skills a must; bilingual (Spanish) preferred. Criminal background check & drug screening required.

HOURLY RATE:  $110.00 – $120.00

NURSE MANAGER – Full-time

SUMMARY:  Provide general nursing care to patients; assist provider with examinations and procedures; complete assessments, provide physical and psycho-social support to patients; provide training, orientation and education to staff; manage the interdisciplinary care team; participate in QA and clinical committees.

 QUALIFICATIONS:  Bachelor’s Degree in Nursing from an accredited college or university, Master’s Degree preferred; Valid NYS Registered Nurse License; knowledge of nursing care in accordance with regulations and practices established by the NYS Board of Nursing; 2-3 years of primary care experience; proficient in electronic health record (EHR); BCLS certification;  leadership and administrative skills required; bilingual (Spanish) a plus.

 ANNUAL SALARY:  Negotiable, Paid Benefits (Medical, Dental, Life Insurance, Disability, Paid Time Off).

REGISTERED DIETICIAN – Per-diem

SUMMARY:  Provide assessments, care planning, medical nutritional therapy; participate in interdisciplinary care team, QA and clinical committees as appropriate.

 QUALIFICATIONS:  Master’s Degree in Nutrition from an accredited college or university; valid NYS License-Registered Dietitian; 2 years of acute care experience; proficient in electronic health record (EHR); strong interpersonal and communication skills required; bilingual (Spanish) a plus.

HOURLY RATE:  $25.00 – $35.00

LICENSED MASTER SOCIAL WORKER – Per-diem

 SUMMARY:  Provide assessments, mental and substance use evaluations, diagnosis and treatment planning, social services to patients and their families, individual and group sessions; participate in QA and other clinical committees as appropriate.

QUALIFICATIONS:   Master’s Degree in Social Work from an accredited college or university; valid NYS License – LMSW; 1-2 years of acute care experience; proficient in electronic health record (EHR); strong interpersonal and communication skills required; bilingual (Spanish) preferred.

 HOURLY RATE:  $25.00 – $35.00
